Title 40.1-51.1(a), Code of Virginia: Employer did not furnish employment,
and a place of employment which
were free of recognized hazards that were causing, or likely to cause
death or serious physical harm to
employees in that: Employee(s) was exposed to fall hazards while
performing job related duties when
employee(s) was permitted to work from a portable extension ladder laid
horizontally across forks and lifted by
the forks of a Sky Trak 8042 forklift. The Sky Trak 8042 forklift should
not be used to elevate personnel,
unless a compliant work platform is utilized.
One feasible, and acceptable abatement method among others to correct this
hazard is to comply with the
operators manual, and placard at operators controls that read: Use a
compliant work platform to lift and
lower personnel.
