(a)Jet P.I. LLC, at 1935 West 12th Avenue, Denver, CO: On 9/18/07, Jet P.I.
LLC did not furnish employment and a place of employment which were free
from recognized hazards that were causing or likely to cause death or
serious
physical harm to employees, in that one employee was exposed to a high
pressure hydrogen peroxide hazard.  The employer used a stainless steel
cylinder to contain 89hydrogen peroxide.  Compressed nitrogen was used to
pressurize the hydrogen peroxide in excess of five hundred psi.  The
employer
did not ensure that the cylinder used to contain the high pressure hydrogen
peroxide was inspected and approved for containing the hydrogen peroxide at
high pressure.  This condition exposed one employee to a high pressure
hydrogen peroxide hazard.
Abatement Note:  Abatement certification and documentation are required
for
this item (See
enclosed "Sample Abatement-Certification Letter").
Abatement Note:  Among other methods, feasible abatement methods include:
1.Hydrotesting and certification of the stainless steel cylinders for
containment of high
pressure and concentration (approximately eighty nine percent) hydrogen
peroxide by
a qualified engineer.
2.The inspection process is outlined in ASME Boiler and Pressure Vessel
Code, Section
VIII "Rules for Construction of Pressure Vessels".

(a)Jet P.I. LLC, at 1935 West 12th Avenue, Denver, CO: On 9/12/07, Jet P.I.
LLC did not furnish employment and a place of employment which were free
from recognized hazards that were causing or likely to cause death or
serious
physical harm to employees, in that employees were exposed to a fire
hazard.
One employee performed distillations of hydrogen peroxide at approximately
70 degrees Celsius using a vacuum pump.  The distillation was performed to
concentrate the hydrogen peroxide from approximately fifty percent to
eighty
nine percent.  The heating element for the distillation was controlled by a
thermostat that was not interlocked with the vacuum pump.  Loss of vacuum
would not result in an automatic shut down of the heating element.  The
room
in which the distillation took place did not have an automatic sprinkler
system.
The vacuum system did not utilize a cold trap to prevent potential hydrogen
peroxide vapor from entering the vacuum piping or pump.  The overpressure
relief did not have offset vents, which would prevent hydrogen peroxide
liquid
from escaping the flask.  The glassware was cleaned with acetone and
distilled
water, which are reactive with hydrogen peroxide.  Eighty nine percent
concentration hydrogen peroxide at elevated temperature may cause an
explosion or fire if allowed to come into contact with incompatible
materials.
The employer did not ensure that the distillation apparatus and the room in
which the distillation took place were designed and constructed to prevent
an
explosion or fire involving hydrogen peroxide.  This condition exposed
employees to a fire hazard.

1.Obtain 89hydrogen peroxide from a commercial source.
2.Ensure compliance with National Fire Protection Association Chapter 430
