Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard which could inflict death or serious injury while changing out the core on the No. 21 Metal Prep Line: (a)Eastman Kodak Co. Kodak Colorado Division, 9952 Eastman Park Drive, Windsor, CO: On or about 3/08/2008 an employee operated the coil buggy on the No. 21 Metal Prep Line while another employee was positioned in a danger zone associated with the operation of the coil buggy when replacing a rewind core. This exposed the employee to the hazard of being crushed between the full coil on the buggy and the rigid winder gudgeon. Abatement Note: Among other methods, feasible and acceptable abatement methods would be to implement engineering and/or administrative controls to prevent the coil buggies on the No. 21 Metal Prep Line from being operated while employee(s) are positioned in the danger zone associated the operation of the buggies. (b)Eastman Kodak Co. Kodak Colorado Division, 9952 Eastman Park Drive, Windsor, CO: On or about 3/08/2008 and at times prior thereto employee(s) who operated the "coil buggy" at either end of Metal Prep Line 21 were not sufficiently trained to avoid operation of the buggies while other employee(s) are in the danger zone. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Sample Abatement-Certification Letter").
