furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that:
On or about November 9, 2006, employees were exposed to crushing injuries
due to the
unexpected release of energy when entering the crushing chamber of the
Hartl Impact Crusher
PC 12/70, SN 724 120 074, to clear jammed pieces of concrete, rock, other
aggregate or
building materials without blocking, restraining or otherwise rendering
the rotary crushing drum,
hammer(s), inner walls or other equipment inoperable or immobile from all
mechanical,
hydraulic or other sources of energy or where the release of stored energy
or movement could
cause the drum to turn or otherwise create a danger to employees.
Among other methods, one feasible and acceptable abatement method for
correcting
these
hazards would be to require employees to follow the manufacturer's
recommendations for
performing service work on the Hartl Impact Crusher by the manual provided
by Hartl
Anlagenbau GmgH including, but not limited to: powering down and securing
the rotor/impact
hammers from movement; bracing or otherwise securing the rotor cover from
dropping; ensuring
that the diesel motor is off, secured/locked out to prevent accidental
start-up; ensuring that the
clutch is disengaged; ensuring that the main switch is off and that power
to the switch is
completely isolated and locked out to prevent accidental startup; and
testing/verifying that all
energy sources have been effectively disabled.ly
