Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck-by/fall hazards due to the geronimo excape line not
being properly
installed:
a)  On or about May 27, 2007 and at times prior, Cade Drilling Co. LLC did
not ensure
that employees were protected from struck-by/fall hazards.
1.  The Geronimo excape device was not connected at the monkey board in a
manner which the employees could safely and quickly mount and operate.
2.  The wire rope of the excape device was not long enough to both reach
far
enough from the rig and have enough slack to allow for a slow down section.
3.  The wire rope was not properly attached to the ground anchor in that
the
chain, it was connected to, stretched to a point 10 feet off the ground
where the
user would have had to jump off or worse fallen at a high rate of speed.
Among other methods, one feasible and acceptable abatement method to
correct the hazard is
to ensure proper erection and guying procedures according to the
manufacturer's specific
instructions.

and the air hoists:
a)  On or about May 27, 2007 and at times prior, Cade Drilling Co. LLC did
not ensure
that employees were protected from struck-by hazards.
1.  The crosby clamps used to form the eye in hoisting lines were not
installed
according to manufacturers specifications.  The clamps were not torqued to
proper levels, were not properly spaced on the line, and proper numbers of
clips
were not used to make the connections.
2.  The wire rope used for the hoist line was worn, showing multiple broken
strands and severely worn strands.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to install the Crosby clamps according to manufacturers specifications and
to replace the wire
rope when it reaches industry, remove from service, criteria listed in
American Petroleum
Institute Recommended Practices (API-RP 54-9.6)
