Section 5(a)1 of the Occupational Safety and Health Act of 1970:
The employer did not furnish to each of his employees employment and a
place of
employment which were free from recognized hazards that were causing or
likely to cause
death or serious physical harm to employees in that employees were exposed
to a fall hazard:
On or about 01/30/08, in the warehouse, an employee was standing on the
forks of a Toyota
forklift in a elevated position approximately 16 feet above the ground
floor.
Among other methods of abatement, one feasible and acceptable method to
correct the fall
hazard is to prohibit employees from standing on the forks of powered
industrial trucks.

REQUIRED FOR THIS VIOLATION.
