Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and/or amputation hazards due to inadequate training and inspections: (a) Location - 12 Hickory Lane, North Reading, MA: On or about 8/28/07, the employer failed to: 1) Establish and follow a program of periodic and regular inspections of all equipment 2) Provide employees with instruction and training to recognize defective and/or unsafe equipment or conditions and methods to report such unsafe equipment or condition 3) Ensure that all service opening covers and access doors are maintained and in place while vehicle is in use 4) Protect employees from any pinch point at the point of operation during the packing cycle 5) Protect service openings that expose pinch points with a cover to prevent inadvertent entrance and 6) Ensure that riders maintain four point contact while positioned on the riding step. Among other methods of abatement, one feasible and acceptable means of abatement is to follow the most applicable version of American National Standards Institute (ANSI) Z245.1 requirements for Mobile Wastes, Refuse and Recyclable Materials Collection, Transportation, and Compaction Equipment - Safety Requirements. Abatement certification must be submitted for this item.
