Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment which
were free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that employees were exposed to live electrical components in
wet or damp
locations that were either in a state of disrepair or not designed for use
in that environment.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS:
(a)  Develop and implement written procedures requiring that prompt
corrective action
is taken any time that an electrical issue is identified.
(b)  Develop and implement a disciplinary action program and procedures to
ensure
that employees report damage as it occurs and take action to ensure that
the electrical
fixtures are not abused/neglected.
(c)  Take precautions with regards to the electrical installations as to
prevent an
accident from happening including but not limited to, ensuring all
employees receive
training on the proper methods for working around electrical fixtures.
This should
also include training on the appropriate methods for inspecting the
worksite for related
hazards and then the development of procedures to report the issues.
At the following locations, the employer failed to ensure that live
electrical components in
wet or damp locations were properly maintained and designed for use in
that environment:
a)  Room 3:  Receptacle cover not secured to prevent water from entering
and also
exposes live parts.
b)  Room 4:  Missing globe on a light fixture.
c)  Room 5:  Receptacle cover is damaged.
d)  Room 8:  Receptacle cover is damaged.
e)  Pressure Washer Room:  Electrical cords used in a wet location showed
visible
signs of damage.
f)  Pressure Washer Room:  A space heater used in the pressure washer room
was not
designed for use in wet or damp environments.  The case of the unit showed
visible
signs of corrosion.
g)  Pressure Washer Room:  A receptacle had arc marks from water exposure
and
inappropriate use.
h)  Electrical Room:  Sub Panel 1 was missing a breaker which exposed the
live partswithin.
i)  Room 11:  A receptacle mounted on the ceiling was missing a cover and
have
visible damage (melting) due to electrical arcing.
j)  Room 12:  Receptacle cover is damaged.
k)  Room 13:  Receptacle cover is damaged.
l)  There was not any type of ground fault circuit interrupter to protect
the employees
from the hazards of electrical shock in the presence of water or some
other type of
ground fault exposure.
Abatement certification and abatement documentation is required for this
violation. The
documentation should include written verification of abatement, applicable
measurements or
monitoring results, and photographs or videos which you believe will be
helpful. The
abatement certification sheet is enclosed with the citations.

physical harm to
employees in that unqualified personnel are performing electrical work at
the site.  This
exposes not only the person performing the electrical work to the hazards
of working with
electricity but also the employees in the area to any potentially
incorrectly installed fixtures.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS:
(a)  Develop and implement written procedures requiring that prompt
corrective action
is taken any time that an electrical issue is identified.
(b)  Develop and implement a disciplinary action program and procedures to
ensure
that only qualified persons to perform electrical work.
(c)  Ensure that only persons meeting the National Electric Code
definition of a
qualified perform electrical work at the site.  This would also include
the
utilization
of a lockout/tag out program to protect that person while the work is being
accomplished.
Abatement certification and abatement documentation is required for this
violation. The
documentation should include written verification of abatement, applicable
measurements or
monitoring results, and photographs or videos which you believe will be
helpful. The
abatement certification sheet is enclosed with the citations.
