which were free from recognized hazards that were causing or likely to
cause death or serious
physical harm to employees in that employees were exposed to:
In that the employer did not provide a safe work place for employees
operating a Hyster forklift
on the docks of the shipping and receiving department.  The Hyster
forklift model E50XM serial
number F108V17674W had half of the seat belt removed from the forklift.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to install a new seat belt on the forklift and ensure employees use it.
