29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1
m) above a lower
level was not protected from falling to that lower level in accordance
with paragraphs (g)(1)(i)
through (vii) of this section:
a.Job site On or about 7/21/2008, an employee who was working on the
platform
affixed to the forks of an New Holland rough terrain forklift were not
protected
from falls. The employee was not wearing harnesses and the platform did not
have railings (top rail and midrail) on three of the four sides. The
platform was
approximately 12 feet above the ground.
