Missouri- The
employer did not provide rollover protection for an employees working with
compacting roller machines. The Sakai TW320 Vibrating Roller did not have
its'
rollover bar in place for the operator. The Sakai roller's collapsible
ROPS bar
was not functional in that the bar was collapsed and secured to the base
of the
bar.

CERTIFICATION OF
CORRECTIVE ACTION WORKSHEET), and in addition, documentation
demonstrating that
abatement is complete must be included with your certification.  The
documentation
