An employee was fatally injured when he was caught between the back end of
a truck trailer and
the wall of a warehouse. On or about 11/21/08.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS: 1) Develop and implement written
procedures
requiring operators of vehicles to have visual confirmation of the ground
person's location at all
times when the vehicles are in motion. The procedures should require
operators to exit their
vehicles and make sure the path is clear before backing. 2) Develop and
implement a form of
communication method to be used by drivers and the ground persons. This
can
be through hand
signals or radio communication. 3) Develop and implement a disciplinary
action plan to ensure
that employees follow the procedures. 4) Review and audit training
programs and trainers to
ensure that information being presented is factual and correct as per the
latest regulatory
directives and reflects Occupational Safety and Health Administration
(OSHA), Department of
Transportation and company policy.
Abatement certification and documentation is required for this violation.
The documentation
should include written verification of abatement, applicable measurements
or monitoring results,
and photographs or videos which you believe will be helpful.
