nozzle while working in
a confined space: TransCanada Site, Rhode Island: a wand capable of
operating at 300 psi was
not provided with a manually-actuated control (deadman control) while
hooked up to the ColdJet
delivering dry ice equipment.
Abatement Note: Among other methods, one feasible and acceptable abatement
to correct this
hazard is to require a manually operated deadman control, with the
actuator trigger capable of
shutting off the dry ice flow when released.

employees who were exposed to confined space hazards while working in the
HRSG #1 stack,
on or about April 9, 2008, were not provided with adequate management
oversight to ensure
compliance with the employer's confined space entry program.
Abatement Note: Among other methods, feasible and acceptable abatement
methods to correct
this hazard include providing adequate management oversight to ensure that
the following
precautions are being implemented:
1.  Testing or monitoring permit spaces as necessary during entry
operations.
2.  Reviewing the permit-required confined space program using canceled
permits within
one year after each entry.
3.  Ensuring that the entry permit contains all required information,
including the type
of communication means used.
4.  Ensuring that each attendant remains outside the permit space during
entry operations
until relieved by another attendants.
5.  Ensuring that each attendant communicated with the entrant using
adequate
communication means.
6.  Ensuring that the attendant does not perform duties which interfere
with their primary
duty to monitor the entrant.
7.  Ensuring that all employees are adequately trained to perform their
assigned duties.
