Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to THE HAZARD OF BEING STRUCK-BY:
a)Dismantling Shop Bay/Lift #1 and 2, on or about 03/09/2009: Two vehicle
lifts were being used to lift automobiles for dismantling used auto parts
and
following hazards were observed:
1)Bay/Lift # 1: Dismantlers were working under the loaded vehicle lift
with non-functioning safety devices in that the Right side Swing lift
arm link adjuster was removed.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazards are to:
1.Utilize all safety features provided on the vehicle lift, and operate the
lift in accordance with the instructions furnished by the lift
manufacturer. ANSI/ALI ALOIM-2000, Safety Requirements for
Operation, Inspection and Maintenance, Section 4 and Challenger
Lifts Inc's Manufacturer's Manual, Operation, Installation &
Maintenance.
2.Ensure that operators operate the vehicle lifts only after being
properly instructed or trained and maintaining training documents in
accordance with ANSI/ALI ALOIM-2000, Safety Requirements for
Operation, Inspection and Maintenance, Section 4 and Challenger
Lifts Inc's Manufacturer's Manual, Operation, Installation &
Maintenance
3.Ensure that a qualified lift inspection is conducted no less than
annually in accordance with ANSI/ALI ALOIM-2000, Safety
Requirements for Operation, Inspection and Maintenance, Section 5
and Challenger Lifts Inc's Manufacturer's Manual, Operation,
Installation & Maintenance.
4.Ensure that qualified maintenance personnel conduct required
maintenance and all service and repairs be documented and
maintained in accordance with  ANSI/ALI ALOIM-2000, Safety
Requirements for Operation, Inspection and Maintenance, Section 6
and Challenger Lifts Inc's Manufacturer's Manual, Operation,
Installation & Maintenance.
5.Ensure that operating instructions, general safety information, safety
tips and warning labels are displayed in a conspicuous location in the
lift areas in accordance with ANSI/ALI ALOIM-2000, Safety
Requirements for Operation, Inspection and Maintenance, Section 4
and Challenger Lifts Inc's Manufacturer's Manual, Operation,
Installation & Maintenance.
b)Dismantling Shop Bay # 3, on or about 03/09/2009: A 1/2 Ton electric Duff
Norton Overhead hoist was being operated on a monorail with the following
observed hazards:
1)The hoist was in use to lift an automobile engine for dismantling with
the safety latch removed from the hoist hook.
2)The hoist was being used on an 'A' Frame monorail system which was
not rated or marked with the lifting capacity.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazards are to:
1)Replace the safety latch on the hoist hook as designed by the hoist
manufacturer in accordance with ANSI/ASME B30.16 Overhead
Hoists (Underhung) Section 16, Inspection, Testing, and Maintenance.
2)Rate and mark the rated load capacity on the monorail system in
accordance with ANSI B30.11 - 1993, Monorail systems and
underhung cranes, Section 11, Construction and Installation.
Abatement certification must be submitted for these items.
