Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm:
Davenport Prue Sand Unit Well #133; on or about April 16, 2009, employees
were engaged in
the operation of a well service rig to replace a broken (parted) sucker
rod, they had pulled the
sucker rod to the break storing it in the rig mast rod rack; the rig
stabilizing guylines were not
attached to anchors leaving the rig stabilized only by the carrier
supports and guylines attached
to the carrier front.  This exposed all the employees to the hazards of a
rig overturning as being
struck by rig and well components, and falling from mast elevations
upwards of 80 feet.  The
American Petroleum Institute (API) Recommended Practice (RP) 54,
Recommended Practice for
Occupational Safety for Oil and Gas Well Drilling and Servicing
Operations, 6.3.2 requires that
the rig be rigged up in a safe manner before starting well operations with
the rig, and 9.2.10
requires that the rig guylines be properly tensioned prior to imposing a
load on the derrick or
mast.  Among other methods, one feasible and acceptable abatement method
to correct this
hazard is to assure that the rig is safely rigged up prior to the
imposition of any operational load
on the rig mast and that the guylines are properly tensioned to certified
anchors.d

working in the 80 foot high rod basket manually racking the sucker rods;
the derrick hand was
not provided a secondary means of emergency egress from the mast.  This
prevented the derrick
hand from making timely emergency egress from the mast during rig
emergencies; potential
emergency hazards include: fire, explosion, flying objects and rig
collapse.  The American
Petroleum Institute (API) Recommended Practice (RP) for Occupational
Safety for Oil and Gas
Well Drilling Servicing Operations, RP 54, 6.10.1 requires that an
auxiliary
means escape be
installed prior to the derrick hand working on the mast.  Among other
methods, one feasible and
acceptable abatement method to correct this hazard is to assure that the
escape system is installed
in accordance with the manufacturer's instructions prior to the placement
of the derrick hand in
the mast.
