Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing and struck- by hazard:
a) On or about 10/7/08 in the challenger lift area, employer did remove a
Challenger lift model
36000 serial #360750 from service after one of the locking devises broke
off the previous week,
exposing employee to crushing/struck by injuries from unstable loads while
doing inspection and
changing tires.
Among other feasible methods and acceptable abatement method to correct
this hazard is to
follow (1) ANSI/ALOIM-2000, (2) follow Challenger Lift Inc operation,
installation &
maintenance manual. Hazardous condition was abated during inspection by
removing car from
lift and locking lift out of service and instructing employee lift can no
longer
be used until lift
is repair.
