Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to multiple injuries and or death from falls created by using an inappropriate personnel work platform attached to a rough terrain forklift truck: a)One employee was exposed to a fall hazard created by performing roofing work from an elevated work platform attached to a Pettibone B68 Extendo rough terrain forklift truck; this platform was not marked with a workload or empty weight marking placard and had floor dimensions greater than the overall width of the truck (measured across load bearing tires plus 10 in). Among other methods, feasible and acceptable abatement methods to correct this hazard are to ensure the work platform is designed and constructed according to the acceptable requirements of the approved American National Standard, Safety Standard for Rough Terrain Forklift Trucks, ASME B56.6-2002, Part III, Section 8.24 Platforms for Elevating Personnel and other acceptable industry practices. The ASME consensus standard requires the following: (1) Section 8.24.1 Design requirements for the manufacture of the platform shall include the following: (b) Floor dimensions shall not have a width greater than the overall width of the truck [measured across the load bearing tires plus 10 in. (250mm) on either side]. (f) Information prominently indicated on the platform: (1) maximum work load including personnel and equipment; (2) weight of empty platform. Abatement Certification or Abatement Documentation required.tion
