safeguards, and did not adopt
and use such practices, means, methods, operations and processes as was
reasonably necessary
to render such employment and places of employment safe and healthful, and

other thing reasonably necessary to protect the life, safety, and health
of the employees:
a) Employees were exposed to being struck-by vehicles while conducting
flagging operations in
a complex traffic/pedestrian intersection located at Interstate and
Holiday near the Rose Quarter
in Portland, Oregon. Employees were not trained/certified as flaggers, did
not have advanced
warning devices to warn motorists of flaggers in the roadway, and were
flagging in conflict with
traffic signals.
