Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck by hazards.
a)North Side Of Building: The company was using a 3/4" by 30' Crosby two
leg
bridle with shackles to off load steel that lacked a manufacture's
identification,
on or about 3/4/09.
Note: Among others, feasible and acceptable methods to correct this hazard
include but are not limited to:
Purchase rigging hardware from a known manufacturer that complies with the
standards or have rigging proof tested by a qualified engineer and
document the
results.
NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT
CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN
AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
