29 CFR 1910.119(f)(1)(i)(D): The employer's written operating procedures covering the steps for each operating phase did not address emergency shutdown including the conditions under which emergency shutdown is required, and the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown in a safe and timely manner: On or about April 29, 2010, at WRB Refining, the employer did not ensure that written emergency shutdown procedures provided clear instructions for the emergency depressuring of the S-Zorb in the event of a fire. The written procedures did not address all of the steps involved when depressuring the reactor: a.The procedures did not include the opening of the bypass when the warm product separator emergency depressuring valve failed to open. b.Pressure was also relieved through the Cool Product Separator and not included in the emergency shutdown procedures. ***ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM IN ACCORDANCE WITH THE REQUIREMENTS OF 29 CFR 1903.19(d)***
