29 CFR 1910.119(d)(3)(i)(B):  Process safety information pertaining to the
equipment in the
process did not include the piping and instrument diagrams (P&ID'S):
At WRB Refining, the company did not ensure the Piping and Instrumentation
Diagrams were
correct and up to date. On DWG E48395, the 2 inch bypass line coming off
of the Warm
Product Separator had two block valves; one was labeled car-sealed closed
(CSC) while the other
valve downstream was not labeled on the P&ID as car-sealed open (CSO).

29 CFR 1910.119(f)(1)(i)(D):  The employer's written operating procedures
covering the steps
for each operating phase did not address emergency shutdown including the
conditions under
which emergency shutdown is required, and the assignment of shutdown
responsibility to
qualified operators to ensure that emergency shutdown in a safe and timely
manner:
On or about April 29, 2010, at WRB Refining, the employer did not ensure
that written
emergency shutdown procedures provided clear instructions for the
emergency depressuring of
the S-Zorb in the event of a fire. The written procedures did not address
all of the steps involved
when depressuring the reactor:
a.The procedures did not include the opening of the bypass when the warm
product
separator emergency depressuring valve failed to open.
b.Pressure was also relieved through the Cool Product Separator and not
included
in the emergency shutdown procedures.

outside acceptable limits (as defined by process information in 29 CFR
1910.119(d) before
further use or in a safe and timely manner:
On or about April 29, 2010, at WRB Refining, in the S-Zorb Unit the
employer did not correct
deficiencies in equipment before further use or in a safe and timely
manner:
a.Misalignment of outlet piping coming from the reactor effluent filter
was not
corrected to prevent the escape of gasoline;
b.Bolts were not replaced on the outlet flange on the reactor effluent
filter in a
timely manner to prevent fire or loss of gasoline.
