29 CFR 1910.119(d)(3)(i)(A):  Process safety information pertaining to the
equipment in the
process did not include the materials of construction:
On or about May 7, 2009, the employer did not ensure that process safety
information related
to the materials of construction of the expansion joint in the PCLA-3 CO
Furnace/Wet Gas
Stripper (F301) was accurate and complete. This exposed employees to an
asphyxia hazard
related to Carbon Monoxide when it was released from a ruptured expansion
joint attached to
the CO Furnace.
Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of this
citation, the
employer must submit documentation showing that it is in compliance with
the standard,
including describing the steps it is taking to ensure that the process
safety information addresses
the materials of construction of the expansion joint.

hazards of the
process:
On or about  May 7, 2009, the employer did not ensure that the PHA's
concerning the PCLA-
3/CO Furnace/Wet Gas Stripper included the potential release of Carbon
Monoxide (CO) into
the unit.  Employees were exposed to an asphyxia hazard when CO was
release from a ruptured
reinforced rubber expansion joint that was attached to the CO Furnace.
Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of this
citation, the
employer must submit documentation showing that it is in compliance with
the standard,
including describing the steps it is taking to ensure that the PHAs
addresses the release of CO
into the unit.

administrative controls applicable to the hazards and their
interrelationship, such as, appropriate
of detection methodologies to provide early warning of releases.
On or about May 7, 2009, the employer did not ensure that the PHAs
concerning the PCLA-
3/CO Furnace/Wet Gas Stripper included engineering and/or administrative
controls to detect
the release of Carbon Monoxide (CO) from the CO Furnace.  Employees were
exposed to an
asphyxia hazard when CO was released from a ruptured reinforced rubber
expansion joint
attached to the furnace.
Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of this
citation, the
employer must submit documentation showing that it is in compliance with
the standard,
including describing the steps it is taking to ensure that the PHAs
addresses the appropriate early
warning detection systems to warn employees of the release of CO into the
unit.
