ON A SCAFFOLD 32 FEET FROM THE GROUND WERE MANUALLY HOISTING A 248
POUND DEBRIS CHUTE WHICH WAS TO BE INSTALLED ON THE END FRAME OF
THE SCAFFOLD. DURING THE MANUAL HOISTING OF THE CHUTE, THE CHUTE
UNEXPECTEDLY DROPPED, EXPOSING FOUR WORKERS TO LIFTING LINE
ENTANGLEMENT, CRUSHING AND STRUCK BY INJURIES.
2. USE A CRANE OR OTHER EQUIVALENT MECHANICAL LIFTING DEVICE TO
INSTALL THE CHUTE.
