Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: (a)Rex Industries Inc, at 6513 Weld County Road 17, Fort Lupton, CO: On 7/20/09, Rex Industries Inc did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard. Employees were removing an antenna from a telecommunications tower. Employees worked to a height of approximately one thousand feet above ground level. Employees used a base-mounted drum hoist to lower employees from approximately 1000 feet in height to ground level. Specific actions were not taken to ensure the base-mounted drum hoist was approved for personnel hoisting. (b)Rex Industries Inc, at 6513 Weld County Road 17, Fort Lupton, CO: On 7/20/09, Rex Industries Inc did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard. Employees were removing an antenna from a telecommunications tower. Employees worked to a height of approximately one thousand feet above ground level. Employees used a base-mounted drum hoist to lower employees from approximately 1000 feet in height to ground level. The employer did not perform a trial lift prior to personnel being lowered by the hoist. The wire rope was of insufficient length to reach the ground and the wire rope unspooled from the drum when the friction wedge failed to secure the wire rope. The employer did not perform a trial lift prior to personnel being lowered by the hoist to ensure the hoist operation would perform safely. Abatement Note: Among other methods, feasible and accepted means to correct the hazardous condition include compliance with ANSI B30.7 "Base-Mounted Drum Hoists", National Association of Tower Erectors (NATE) "Guidelines for the Radio, Television, Communications Tower Industry", and OSHA Directive CPL 01-01-036 "Interim Inspection Procedures During Communication Tower Construction Activities". Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter").-Certification
