Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck by, fall and flying object hazards:
a)On June 15, 2009, at the site located at 2424 Dempster in Evanston,
Illinois 60202, Joel
Martinez dba Abraham Roofing Company provided a Spar-Marathon "A" Frame
Hand
Hoist that was not equipped with approved counterweights.
Among other methods, a feasible and acceptable mean of abatement is to:
1) Use
counterweight materials specified by the hoist manufacturer.
**In accordance with 29 CFR 1903.19(c), abatement certification is
required for this violation
(using the CERTIFICATION OF CORRECTION ACTION WORKSHEET).**
