Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that: they are exposed
to crushing hazards.
Employees were exposed to crushing hazards when utilizing the automotive
lift in bay
#4 in that:
(a)The employer failed to ensure that the safety latch on the rear jack
lift was
operating properly.  This exposed employees to crushing hazards.
(b)The employer failed to ensure that the pneumatic system which controls
the front
lift pneumatic jack was operating correctly.  This exposed employees to
crushing
hazard when the lift would operate without being actuated by controls.
(c)The employer failed to institute an inspection schedule for the
automotive
lifts to
ensure that they were proper working order.  This exposed employees to
crushing
hazards form the malfunctioning lifts.
Among other methods, feasible means of abatement by which the hazards may
be
corrected are as follow:
1.Establish compliance with ANSI/IALI B153.1-1990, "American National
Standard
for Automotive Lifts - Safety Requirements for the Construction, Care, and
Use."
2.Inspected lift to determine any malfunctioning parts and have a schedule
for
ensuring safety devices are functioning.
3.Repair or replace the malfunctioning equipment.
4.Document and develop an inspection procedure and schedule for the
automotive
lifts.
Certification, and documentation/evidence of abatement is required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment

to struck-by hazards from the overhead door.
(a)The employer failed to ensure that the overhead door in Bay #3 was
adequately
constructed and adjusted to prevent it from coming off of its track.  This
exposed
employees to struck-by hazard.
Among other methods, feasible methods of abatement by which the hazard may
be
corrected are as follow:
1.Establish compliance with American National Standards Institute/Door &
Access
Systems Manufactures' Association, International's, "Standard Method for
Testing
Garage Doors:  Determination of Life Cycle Performance" (ANSI/DASMA 109-
2007).
2.Provide additional support to the track system.
3.Reduce the spring tension on the door, too allow the door to go up at a
lower
speed.
Certification, and documentation/evidence of abatement is required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment
