Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm:
East side of the well head; on or about July 22, 2009, an employee
operated a high pressure
pump truck in an injection well repair operation to find a tubing leak,
the truck high pressure
pump discharge was not protected by a relief valve.  The pump output
pressure was rated to
5000 PSI and was being operated at 2700 PSI.  This exposed the employee to
the hazards of
being struck by high pressure fluids and/or fluid propelled objects.
Among other methods, a
feasible and acceptable abatement method to correct these hazards would be
to install a pressure
relief valve in accordance with the pump manufacturer operating and
service manual for the
pump.l
