Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm from injury in that, employees were exposed to being asphyxiated from hydrogen sulfide: a) On or about May 13, 2010, and at times prior, employees were exposed to injury in that, employees were exposed to being asphyxiated from hydrogen sulfide in that an adequate hydrogen sulfide program had not been implemented: Among other methods, one feasible and acceptable method to correct this hazardous condition would be to implement a hydrogen sulfide program which complies with API RP68. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm in that, employees were exposed to chemical injection hazards: a) On or about May 13, 2010, and at times prior, employees were exposed to injury while operating the Tongs that had worn hydraulic hoses. Among other methods, one feasible and acceptable method to correct this hazard condition would be to implement an hydraulic hose inspection program in accordance with the manufactures recommendations, and ensure all hose that fail the inspection are replaced immediately. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification

Among other methods, one feasible and acceptable method to correct this hazard condition would be to follow API 4G 13.3 the manufactures recommendations for torque clamps on guylines. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification
