29 CFR 1910.178(p)(1):  Powered industrial truck(s) with defect(s) or in
any way unsafe had
not been withdrawn from service until restored to safe operating
condition(s):
a)  Guardsmark, LLC on or about July 25, 2010, at the worksite located at
1161 Buckeye Rd.
in Lima, Ohio:  The employer did not assure that the Cushman powered
industrial truck (s/n
1CUNH2220JL014571) was removed from service during a time in which the
tires were found
to be grossly under inflated.  Under inflated tires may inhibit an
operator's ability to control the
vehicle.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
( using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence, or
written
records.
