on process equipment to maintain its mechanical integrity did not identify
the date of the
inspection or test, the name of the person who performed the inspection or
test, the serial
number or other identifier of the equipment on which the inspection or
test was performed, a
description of the inspection or test, and the results of the inspection
or test:
Establishment:
On or about February 5, 2010, the following inspection checklists did not
contain all the
information required by this section:
a) Purger written checklist did not list the day that the inspection
occurred.
b) Air-Cooled Condensing Unit written checklist did not list the day that
the
inspection occurred.
c) Compressor Rack Package written checklist did not list a date or the
name of
the employee who conducted the inspection.
