Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to potential amputation hazards:
a) Fabrication and Machining Areas- The employer failed to ensure that all
metal cutting saws
such as the Jet Vertical Bandsaw, Serial #6051171C, the Delta Vertical
Bandsaw in Machining,
and the W. F. Horizontal Bandsaw in Machining, were equipped with a
mechanism to prevent
the machines from automatically restarting upon restoration of power after
a power failure.
Among other methods, one feasible and acceptable method to correct this
hazard is to
include, but not be limited to the following:
1) Provide a magnetic restarter or similar device as required by the
American National
Standards Institute (ANSI) B11.10-2003, Section 6.11, that in the event of
an interruption
in the electrical supply the device would prevent the saw from restarting
automatically
when power is restored.ally
