There was no evaluation performed to determine the possible hazards of changing from cotton uniforms to Pyrolon CRFR uniforms and from rubber gloves to nitrile gloves. Condition noted on or about 4/12/2010.

An incident investigation was performed and completed on 5/10/2010 but was not reveiwed with all affected employees. Condition noted on or about 9/21/2010.

29 CFR 1910.303(g)(1)(ii): Working space about electric equipment rated 600 volts, nominal, or less was used for storage: Equistar Chemicals Edison NJ Site - a) Maintenance Shop/ Welding Area: Electrical panel boxes/ junction boxes rated 480V/3 phase and 220V single phase that controlled a welding machine, the exhaust fan and a lathe in the building were not maintained free of storage. The areas were blocked with items including but not limited to machinery, power cords and various other tools. Condition noted on or about 8/26/2010
