were exposed to the hazard of falling:
a) 236 Lafayette Ave. in Edison, NJ:  An employee was exposed to falls
between 7 and
14 ft to the ground while installing Tyvek HomeWrap to the exterior side
of the
southeast wall of a new Sonic Drive-in under construction.  The worker was
standing on
a job-made platform (pallet) on the elevated forks of a Lull telehandler
(Highlander Mdl
844C-42) which was not secured to the forks/carriage and did not
incorporate a guardrail
system.  Furthermore, the worker was not otherwise protected by wearing a
body harness
coupled to a shock-absorbing lanyard as a personal fall arrest system.
Condition(s) noted on or about 9/17/2010.
NOTE: Among the feasible & acceptable methods to correct this hazard is to
follow the
guidance in the Lull Mdl 844C Operators and Safety Manual which strongly
caution
against
lifting personnel, and then only with a properly designed work platform in
accordance with
ASME/ANSI B56.6 Paragraph 5.15.raph
