P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to severe burns from the potential dust explosion, deflagration and fire hazard(s): caused by the improper design and maintenance of dust collection system: a)Cabinet Assembly Area, on or about February 2, 2011, PVC piping or plastic vent tubing was used to connect sections of duct work to wood working equipment, exposing employees fire and explosion hazards. b) Cabinet Assembly Area, on or about February 2, 2011, wood working equipment such as, but not limited to, radial arm saws, table saws, sander and cut-off saw connected to the ventilation system ductwork did not have a type of protection installed to prevent deflagration, propagation of wood dust explosion from traveling between the different equipment connected to the ventilation ductwork ABATEMENT CERTIFICATION AND VERIFICATION IS REQUIRED Among other methods, a feasible abatement method to correct this hazard is all duct work shall be constructed of metal and be bonded and grounded from the equipment to the dust collector. Balance the dust collection systems so that wood dust is captured completely. Install a deflagration vent on the duct work to the outside from the dust collection pipes to reduce or eliminate any flame front from advancing. Perform inspections of the dust collection systems and protect enclosureless dust collection systems with spark detectors. NFPA 664, Prevention of Fires and Explosions in Wood Processing and Woodworking Facilities, 2007 2.2.1.1 states, "Nonconductive ducts such as PVC pipes shall not be permitted. NFPA 664, Prevention of Fires and Explosions in Wood Processing and Woodworking Facilities, 2007 B.6, Deflagration Isolation: Interconnections between separate pieces of equipment present a special hazard, where such interconnection are necessary, deflagration isolation devices should be considered, or interconnections should be vented. Annex D Conveying System Isolation D.1-1 Prevent explosions from propagating back into the production building through the pneumatic conveying duct using a backblast damper and D.1-3 Minimize the chance of conveying burning material into the bag filter dust collector using a high-speed abort gate.
