he employer did not furnish employment and a place of employment which were free
from recognized hazards that were causing or likely to cause death or serious p
hysical harm to employees in that employees were exposed to the hazard(s) of: cr
ushing, and struck-by hazards, when the employer using a concrete dispensing uni
t did not use skid pins, securing it to the lift channels behind the lift forks.
a)Grout Hog, on or about February 3, 2012, the concrete/grout delivery system w
as being used without skid pins in place behind the lift channels securing the h
opper to the lift forks, thus exposing employees working on and around the scaff
old system to struck-by and crushing hazards. ABATEMENT CERTIFICATION IS NOT REQ
UIRED AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS: Ensure employer consult with the forklift or crane manuf
acturer's specifications to ensure load capacity is not exceeded and: 1.Always u
se skid pins and linch pins or safety chains when Grout Hog (e.g. grout delivery
system) is on lift forks. 2.Prior to operating the grout delivery system, slide
lift forks into lifting channels on the Grout Hog, inserting skid pins into lif
t channels behind lift forks and securing with locking pins or use chain attachm
ents or attach to crane using crane bail if equipped.
