Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of getting caught in equipment:
(a) On or about 02/08/2010 - 4493 Redstone Arsenal, Huntsville, AL,
employees
were working in the area in close proximity to a Ditch Witch (Model JT3020)
that was used for the directional drilling operation and a safety buffer
zone of at
least 10 feet was not established.  Warning signs were not placed in the
area.
Among other methods, one feasible and acceptable method to correct this
hazard would
be to establish a buffer zone around the equipment in accordance with the
Operator's
Manual for the Ditch Witch in the Inspection Site Section, Prepare 77 and
provide
warning signs in accordance with "Guidelines for Use of Mini-Horizontal
Directional
Drilling for Placement of High Density Polyethylene Pipe", TR-46, 2009,
published for
the Plastics Pipe Institute.
