Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falls in excess of 14 feet:
On or about 2/2/11, employees at the UT section of 1409 W. Huntingdon St,
Philadelphia, PA, accessed a platform used to hold pallets of records,
which showed
visible signs of stress. The platform's metal grate floor was found
underneath its support
and the clip to keep the grating in place was not operational. This
catwalk holds
personnel and pallets of records with an  estimated weight of
approximately 200 to 300
pounds. The operational capacity of the platform had not been identified.
Among other methods, a feasible and acceptable abatement method would
include the
following:
(1)  Reviewing the design and installation of the grating in the catwalk
to assure it is in
accordance with the grating manufacturer's installation specifications.
(2) Consult with an engineering firm who is certified by the American
Institute of Steel
Construction (AISC) and is Simple Bridge Certified (SBR) to make the
appropriate
repairs to this platform/catwalk.
(3) Follow the requirements of ANSI/ASSE A 1264.1-1989, Safety
Requirements for
Workplace Floor and Wall Openings, Stairs and Railing Systems.
