General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of falling from an elevated personnel platform.
On or about January 27, 2011, two employees were working from a trash box
that was elevated
by a SkyTrak Model #6036 rough terrain, telescoping boom forklift up to
approximately 32 feet
high to apply caulk to the expansion joints on building #8. The box was
used as a personnel
platform that did not meet the manufacturer's requirements for safely
lifting employees with the
forklift. Deficiencies with the platform include:
a) The platform was not designed with a slip resistant surface.
b) Information indicating the maximum work load for personnel and
equipment and the weight
of the empty platform was not displayed.
c) The platform was not equipped with at least one lanyard anchorage point
for each occupant.
Among others, one feasible and acceptable means of abatement would be to
follow the JLG
Industries, Inc. approved fork mounted personnel work platform policy
letter dated February 21,
2007.
Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date
of this citation, the
employer must submit documentation showing that it is in compliance with
the standard,
including describing the steps that it is taking to ensure that employees
are elevated in personnel
platforms that are only approved by the manufacturer.
