Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of being struck-by a falling object:
a)Production Warehouse:  The hook of a Gorbel 2 ton enclosed track
workstation crane (Return #2) was not equipped with a functional safety
latch,
on or about 9/20/2011.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable method to
correct this hazard is to equip the hoist hook with a functional safety
latch as described in
ANSI B30.16-1993 Section 16-1.2.9 (Hooks).  A functional latch shall be
provided to bridge
the throat opening of the hook and retain, under slack conditions, such
items as, but not
limited to, slings and chains.
