Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to an ejection hazard while operating a powered
industrial
truck
without the use of the supplied seat belt:
a)690 Union Blvd, Totowa, NJ: Employee was exposed to an ejection hazard
while operating a powered industrial truck without using the supplied seat
belt,
on or about 2/7/2012.
ABATEMENT NOTE: One feasable and acceptable method of abatement, among
others is to
use the seat belt that was supplied and installed by the powered
industrial truck manufacturer
as stated in the operator's manual, Crown FC450, "Protect Yourself" Page 4.
