29 CFR 1904.4(a):  Each employer required by this part to keep records of
fatalities, injuries,
and illnesses did not record each fatality, injury, and illness that was
work-related, a new case
and meets one or more of the general recording criteria:
Establishment - On or about 1/31/10 an employee received a burn to their
left
outer foot.
The source of the burn was hot stock from the 6th stage bleached washer.
On or about
3/5/10 a prescription medication was applied and the employee lost work
time from
3/6/10 to 3/19/10.  The case was not recorded on the 2010 OSHA 300 Log.
