contract
employee follows the safety rules of the facility including the safe work
practices required by
29 CFR 1910.119(f)(4):
On or about December 8, 2010, the contract employer did not ensure each
contract employee
followed the Holly Refinery safe work practice for entering and exiting
process areas.  Three
JV Industrial Companies employees failed to sign in before entering the
No. 8 Cooling
Tower process area, exposing employees to the potential hazard of fire,
explosion and
inhalation of toxic vapor.
