equipment in the process
complied with recognized and generally accepted good engineering practices:
a.)At the facility: Oil removal from the anhydrous ammonia refrigeration
system
was routed through a two and a half-foot long hose in lieu of two to
four-foot
long rigid piping, thereby employees were exposed to inadvertent
leakage/release of ammonia.

covering safety
and health considerations did not address the precautions necessary to
prevent exposure,
including engineering controls and administrative controls:
a.)Oil Removal Procedure from Ammonia Refrigeration System did not include
the location of nearest eye wash/shower facility, check the ventilation
fans
available in the area, assemble equipment needed such as a clean bucket to
drain oil, necessary hand tools/ wrenches, and clear precautions such as
slowly
open the oil drain valve until a flow of oil starts.

outside acceptable limits (as defined by process information in 29 CFR
1910.119(d)) before
further use or in a safe and timely manner:
a.)Ammonia Machinery Room: Eight pressure safety relief valves (PSRVs)
installed on January 2006, remain in use beyond their scheduled five year
replacement cycles.  These PSRVs were in use on anhydrous ammonia
refrigeration system compressor No. 1, compressor No. 3, and compressor
No. 4.  This condition most recently existed on February 17, 2011.
b.)Ammonia Machinery Room: Four pressure safety relief valves (PSRVs)
installed on August 2004, remain in use beyond their scheduled five year
replacement cycles.  These PSRVs were in use on anhydrous ammonia
refrigeration system compressor No. 3, and compressor No. 4.  This
condition
most recently existed on February 17, 2011.
c.)At the facility: The anhydrous ammonia refrigeration system piping and
system
components were not marked at all locations to identify the ammonia
refrigerant, physical state of the refrigerant, relative pressure level of
the
refrigerant and the direction of flow.  This condition most recently
existed
on
February 17, 2011.
d.)At the facility: Sections of ammonia piping were corroded and pitted.
