Condition:Seatbelt was buckled tight below the seat back.

correct this hazard is
to install seatbelts and have the operators use them as referenced in
ANSI/ITSDF B56.1-V2009-
Safety Standard for Low Lift and High Lift Trucks.
Abatement documentation (photographs, receipts, copies of programs,
training
certification, etc.) is required for this item.

Condition:Eyewash was not connected to the building water supply and the
activating
lever was missing.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to connect the eyewash to the building plumbing and attach a activating
lever as referenced in
ANSI Z358.1-2004 section 5.4.5 and 5.2.
Abatement documentation (photographs, receipts, copies of programs,
training
certification, etc.) is required for this item.
