Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing, or likely to cause, death or serious physical harm to
employees in that employees
were exposed to fall hazards:
On or about February 8, 2011, a Genie GHT 842 powered industrial truck was
used to elevate an employee on a work platform, wood pallet, which did not
meet
the requirements for elevating personnel.  The employee was exposed to a
fall
hazard of approximately 18 feet.
Among others, one feasible and acceptable means of abatement to correct
this
hazard is to provide and use a work platform designed to lift personnel in
accordance with American National Standards Institute (ANSI)/ITSDF B56.6-
2005, Rough Terrain Forklift Trucks, Section 5.15.
