to ensure that periodic inspections were performed on equipment.
c)Manual Machine area: Overhead 3 ton crane, SN #06G31845, and Yale Hoist:
On or about March 1, 2011, the employer exposed to employees to struck by
hazards in that they failed to ensure that periodic inspections were
performed on
equipment to recognize deficiencies, such as the alignment out, rubber
bumpers
are required, wheels have multiple cracks, excessive wear, rail needs
straightened, multiple cuts in control wires, no strain relief, must have
on/off
buttons on pendant, excessive kinks in rope/chain, latch kit broke, brakes
need
replaced, and multiple cuts on hoist festoon.
d)Manual Machine area: Overhead 3 ton crane, SN #333703, and Coffing Hoist:
On or about March 1, 2011, the employer exposed to employees to struck by
hazards in that they failed to ensure that periodic inspections were
performed on
equipment to recognize deficiencies, such as the alignment out, rubber
bumpers
needed, wheels cracked, excessive wear on gears/redeucers and rails/sweeps,
brittle wires/multiple cuts, no strain relief, must have on/off buttons on
pendant,
improper latch kit, brake damage, and multiple cuts in SO cord.
Among others one feasible and acceptable method to correct the hazards is
to follow the
standards in ASME B30.16.2007, section 16-2.1.3(a), periodic inspections
shall
be performed.
