Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to fall hazards:
a)In and around the machine shop:  On March 21, 2011 and at times prior to,
the employer failed to provide mid-rails on a man basket that was elevated
by
a forklift, exposing employees to falls of approximately 10 feet while
installing
lighting in the shop area.
Among other methods, feasible and acceptable abatement methods to correct
this
hazard include, but are not limited to the following:  Ensure that
standard guardrails
are provided for personnel platforms as referenced in ASME B56.1-2004 ( or
ASME
B56.1-2000), Safety Standard for Low Lift and High Lift Trucks, Section
4.17
Elevating Personnel.

a)In and around the machine shop:  On March 21, 2011 and at times prior to,
the employer failed to attach the elevated personnel platform to the forks
or
carriage of the forklift that elevated the basket, exposing employees to
falls of
approximately 10 feet and a struck-by hazard from the basket itself while
installing lighting in the shop area.
Among other methods, feasible and acceptable abatement methods to correct
this
hazard include, but are not limited to the following:  Ensure that
elevated personnel
platforms are securely attached to the forks or lifting carriage as
referenced
in ASME
B56.1-2004 ( or ASME B56.1-2000), Safety Standard for Low Lift and High
Lift
Trucks, Section 4.17 Elevating Personnel.
