Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer does not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a struck-by hazard.
This apparent violation occurred on or about July 19, 2011 near the
generator house.
The Riding Ring lubrication system, operating at 115 psi, had six hoses
attached to tanks
by screw clamps that were not recommended for 115 psi air pressure work.
Among other feasible methods to correct the identified hazard, one method
would be to
replace the high pressure line ends with those recommended by section
19.13.3 of the
American Petroleum institute (API) RP 54 Recommended Practice for
Occupational
Safety of Oil and Gas Well Drilling and Servicing Operations Manual.ons
