Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:05/03/11 Location:Bridge 32 Equipment:TEREX HC110 Crawler Crane Serial #AC 4001 Condition:Employees are exposed to struck by / crushed by hazards by falling materials due to the auxiliary hoist line that was 7/8 inches in diameter reeved on a rooster sheave that had a 1 inch tread diameter. (b)Date of Violation:05/03/11 Location:Bridge 32 Equipment:TEREX HC165 Crawler Crane Serial #AC 4032 Condition:Employees were exposed to struck by/ crushed by hazards by falling material due to the auxiliary and main hoist lines that were 1 1/8 inches in diameter reeved on the boom guide and boom point sheaves that had a 1 inch tread diameter. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the required recommendations as referenced in ASME B30.5-2007 Mobile and Locomotive Cranes, Section 5-1.7.4 (a), Wire Rope Technical Board Wire Rope Users Manual, Inspection of Sheaves and Drums page 55, Bending Wire Rope Over Sheaves and Drums page 51, Crane Tech Lift Equipment Basics Inspector Training Sheave Inspection page 47, Crane Tech Rigging Gear & Sling Inspection, Sheave Inspection page 52 and 53, Mobile Crane Inspection Guidelines for OSHA Compliance Officers 4.0 Inspecting A Mobil Crane Table 4.8A Item 12 Sheaves, page 6 of 12, eLCOSH Managing Mobile Crane Hazards-Crane Selection, Site Review & Setup, page 11 of 15.
