cause death or serious physical harm to employees:

Condition:Employees are exposed to struck by / crushed by hazards  by
falling
materials due to the auxiliary hoist line that was 7/8 inches in diameter
reeved on a rooster sheave that had a 1 inch tread diameter.

Condition:Employees were exposed to struck by/ crushed by hazards by
falling
material due to the auxiliary and main hoist lines that were 1 1/8 inches
in
diameter reeved on the boom guide and boom point sheaves that had a 1
inch tread diameter.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to follow the required recommendations as referenced in ASME B30.5-2007
