Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard: In Bay #2 of the fabrication area; the employer allowed the employees to lift steel products weighing over 4000 pounds using an overhead crane/hoist assembly that was not being inspected periodically. Among other methods, one feasible and acceptable abatement method to correct this hazard is to have the Gaffey/Shaw-box Overhead (Underhung) crane/hoist assembly inspected periodically in accordance with ASME B30.16-2007 Para 16-2.1.3, Overhead Hoists (Underhung).

Among other methods, one feasible and acceptable abatement method to correct this hazard is to have the Gaffey/Shaw-box Overhead (Underhung) crane/hoist pendant control buttons marked or labeled to indicate the direction of motion ASME B30.16-2007 Para 16-1.1.2.a, Overhead Hoists (Underhung).
