Title 40.1-51.1(a) Code of Virginia: The employer, Masters Transmission, Inc., dba: AAMCO Transmissions, did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious bodily harm to employees. The employer did not assure compliance with ANSI/ALI ALOIM - 2000 in the following instances: A) Section 4.4.4, "The lift owner or employer shall take all appropriate steps to follow the recommended inspection procedures of the lift manufacturer, but in no event shall the lift operator fail to inspect or take notice of the following features on a daily basis:" (1) "accessibility and readability of the operating procedures, safety tips and generic safety material;" The operator's manual for the auto lift located closest to the office door was not accessible. Note: One feasible and acceptable abatement method, among others, to correct this hazard is for the employer to provide the operating procedures, safety tips and generic safety material for the lift. B) Section 4.4.4, "The lift owner or employer shall take all appropriate steps to follow the recommended inspection procedures of the lift manufacturer, but in no event shall the lift operator fail to inspect or take notice of the following features on a daily basis:" (7) "damage or excessive wear on any of the lift contact points which engage the vehicle during lifting, whether they engage the tires, frame, body or axles;" The lift located closest to the office door was not equipped with lift adapters (pads) on two of the lift arms. Note: One feasible and acceptable abatement method, among others, to correct this hazard is for the employer to provide lift adapters (pads) for all of the lift arms. C) Section 5.5, "Qualified Inspection Frequency. Inspections by the owner or employer shall follow the recommendations of the lift manufacturer as to frequency. Without regard to the frequency of inspection specified by the lift manufacturer, whether it be weekly, monthly, semi-annually, annually or on some other basis, the owner or employer shall ensure that the points presented in 5.6 are inspected by a qualified lift inspector as a minimum annual requirement." The auto lift located closest to the office door was not inspected at least annually by a Qualified Lift Inspector. Note: One feasible and acceptable abatement method, among others, to correct this hazard is for the employer to have a qualified lift inspector inspect the lift at least annually.
